District Nurses
The Community Nursing Team attached to Church Street Practice offer you an introduction to their work in the community. We work very closely with the other members of the Primary Health Care Team.

What We Do
We visit you in your own home if you are housebound, or too ill to attend your doctor’s surgery or if you have recently been discharged from hospital and need on- going nursing care. You do not need to be referred by a doctor, as a trained nurse will make an assessment of your needs, discuss this with you and draw up a plan of care. We will also discuss when you can expect the District Nurse to visit. Generally one nurse will be allocated to care for you, but we work very closely as a team, so you will be visited by other team members.
Our Aim
We aim to encourage independence and promote self care where possible.
Our Expertise
- Wound care - acute, eg after surgery or minor injury
- Leg ulcer treatment – we also hold a clinic twice a week for people who are able to attend the surgery.
- Management of long term conditions
- Continence care and assessments
- Palliative care - we also care for people at the end of their life, enabling them to remain in a familiar environment with their family, supported by the whole Church Street Team.
- Management of intravenous therapy
- Referral and signposting to other agencies and professionals.
District Nursing is evolving and changing to meet complex needs, reflecting the move to nurse people nearer to their homes. We endeavour to offer a quality service.
